  1. Complete Exhibitor application.  Please write or print legibly.  Don’t forget to put in the credit card information.  When we charge your card (MasterCard, Visa or Discover), a receipt will be sent to you via e-mail.  If paying by check, please make the check payable to NADD and mail to the address listed above.

  2. Complete the Exhibitor contract  (including the 50-word description), sign at the bottom of page two, make two copies of the signed contract and return all to Judie Johnston. 

  3. Complete the Drexel University College of Medicine Exhibitor Contract.  The location is Denver (Broomfield), CO and the dates exhibit will be available are October 17-19, 2012.

Psychotherapy for Individuals with Intellectual Disability
